CNT Dependent Scholarship Form

The Clinics of North Texas offers an annual scholarship in the amount of $1,000.00 to be awarded to a dependent of a CNT employee. Criteria are as follows:

1. The scholarship recipient must be a dependent of a Clinics of North Texas employee.
2. The scholarship recipient must be a graduating high school senior.
3. The recipient must have a grade point average of 3.5 (on a 4.0 scale).
4. The recipient must have received evidence of admission to an institution of higher learning.
5. The recipient must supply a completed Clinics of North Texas application, a cover letter to accompany the application and the evidence of admission to the institution of higher learning.
6. Applications must be turned in no later than July 1st. Applications received after that date will not be considered.

The scholarship will be awarded during the month of August. In the event there are no seniors in a given year, a student who is currently enrolled in an institution of higher learning will be considered for the scholarship. A past applicant will be considered only if there are no senior applicants.
